📋 Description

• Lead, coordinate, and monitor activities for the Rural Health Transformation NY-VISTA Award • Develop and maintain project plans, timelines, milestones, and workstreams aligned with award objectives • Coordinate strategic initiatives and transformation activities across participating organizations and communities • Monitor project progress and identify risks, barriers, and improvement opportunities • Facilitate project meetings, workgroups, and steering committee sessions • Ensure deliverables are completed on schedule and according to grant requirements • Liaise with healthcare providers, community organizations, government agencies, and external partners • Build collaborative relationships with rural healthcare stakeholders • Coordinate communications and provide project updates to leadership and funding entities • Maintain grant compliance and reporting requirements • Track outcomes, performance measures, and KPIs • Prepare reports, presentations, and progress updates • Assist with audits, site visits, and funding-agency documentation requests • Collect, analyze, and interpret project data • Develop dashboards and tracking tools • Identify trends and recommend process improvements • Support continuous quality improvement and outcome evaluation • Monitor expenditures and grant-funding use • Coordinate resource allocation and assist with budget forecasting and financial reporting • Partner with commercial and operational teams on initiative expansion and organizational growth • Lead sustainability planning, funding diversification, and long-term program viability • Coordinate grant development, funding-opportunity identification, application management, and proposal support • Develop and execute stakeholder engagement strategies • Coordinate outreach, communications, and educational content for program awareness and adoption • Identify opportunities to expand partner engagement and participation

🎯 Requirements

• Bachelor's degree in Healthcare Administration, Public Health, Business Administration, Project Management, Communications, or a related field • Minimum of three (3) years of project management experience, preferably in healthcare, public health, rural health, community development, or grant-funded programs • Project Management Professional (PMP) certification or equivalent credential, preferred • Experience managing multiple projects and competing priorities simultaneously • Experience facilitating cross-functional teams and stakeholder collaborations • Experience with federally or state-funded grant programs, preferred • Knowledge of rural healthcare systems, population health, and healthcare transformation initiatives, preferred • Experience with performance improvement and quality management methodologies • Strong project management and organizational skills • Excellent written and verbal communication abilities • Ability to develop and maintain effective working relationships with diverse stakeholders • Strong analytical, problem-solving, and decision-making skills • Proficiency with Microsoft Office Suite, project management software, and data reporting tools • Ability to independently manage priorities and meet deadlines • Knowledge of healthcare regulations, grant administration, and reporting requirements
