📋 Description

• Perform comprehensive medication reconciliation by reviewing and comparing patient medication profiles between the Eirene platform and each client’s Electronic Health Record (EHR) • Identify and resolve discrepancies, including duplications, omissions, or dosing inconsistencies • Collaborate with pharmacists, providers, and care teams to validate changes • Maintain and update participant photo records for Medication Administration Cycle (MAC) programs • Generate and distribute monthly MARs based on client-specific requests and schedules • Verify accuracy prior to distribution and coordinate delivery • Conduct detailed reconciliation of Schedule II controlled substances • Maintain accurate documentation and support audit readiness through consistent tracking and reporting • Oversee and maintain the Long-Term Care (LTC) dashboard within the Eirene system • Provide regular and ad hoc status updates to clients

🎯 Requirements

• High school diploma required or equivalent • Bachelor’s degree preferred • 3 years of pharmacy technician experience is preferred • Must be registered or eligible to become registered with the appropriate State Board of Pharmacy within 60 days of hire • Proficient with MS Office Suite • Excellent typing/data entry accuracy with good speed • Good computer skills to navigate proprietary and other pharmacy software • Strong listening/reading comprehension • Understanding of clinical terms and impacts • Able to multi-task and handle multiple priorities without sacrificing quality • Detail oriented and organized • Independent, responsible and self-directed • Reliable • Efficient • Empathetic and patient focused • Solutions and compliance oriented
