Patients have access to a Board-certified weight loss physician and a Registered Dietitian to develop a tailored plan that encompasses behavioral health, nutrition, physical activity, and FDA-approved medications, if appropriate. Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Provide direct telemedicine care for obesity and comprehensive diabetes management, including insulin titration and patients using insulin pumps or CGMs • Manage an individual patient panel and/or collaborate with and supervise Endocrine APPs and Registered Dietitians • Review charts and conduct initial and return visits • Manage CKD/ESRD and CAD/CV conditions • Provide advanced treatment planning and medical management for complex endocrinology cases • Prescribe non-controlled obesity and diabetes medications • Ensure consistent clinical documentation, protocols, and treatment pathways • Foster colleagues’ clinical growth and support continuing education • Participate in team-based care initiatives • Attend departmental meetings and contribute to quality improvement efforts

🎯 Requirements

• MD or DO degree • Valid and current Board certification in Endocrinology by the American Board of Internal Medicine (ABIM) • 2 years of clinical experience and pharmacotherapy in obesity medicine or endocrinology, post residency, OR completed an accredited Fellowship program in Endocrinology or Obesity Medicine in lieu of years of experience • Active and unrestricted Medical State License in any US State, and eligibility to obtain additional state licenses (IMLC preferred, but not required) • Currently opted-in with Medicare • Competency managing complex co-morbidities, including CKD, ESRD, and CAD/CV conditions • Expertise in advanced diabetes management, including CGMs, insulin pumps, and complex insulin regimens • Experience working with APPs, medical residents, or trainees in a clinical academic setting • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity • Open to working one of the following W2 roles: 32, 36, or 40 hours per week • Excellent communication and interpersonal skills • ABOM certification highly preferred • Synchronous telehealth experience preferred • Medical fluency in Spanish desired but not required • Committed to patient-centered, culturally competent care
