Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Provide direct telemedicine patient care for obesity and comprehensive diabetes management, including insulin titration, insulin pumps, and CGMs • Manage an individual patient panel or collaborate with and supervise APPs and Registered Dietitians specializing in diabetes treatment • Review charts and conduct initial and return visits • Manage CKD/ESRD and CAD/CV conditions and complex endocrinology cases • Provide advanced treatment planning and medical management support for escalated concerns • Prescribe non-controlled obesity and diabetes medications • Ensure clinical documentation, protocols, and treatment pathways are consistently followed • Support colleagues’ professional development through feedback and continuing education • Participate in team-based care initiatives, departmental meetings, and quality improvement efforts

🎯 Requirements

• MD or DO degree • Valid and current Board certification in Endocrinology by the American Board of Internal Medicine (ABIM) • 2 years of clinical experience and pharmacotherapy in obesity medicine or endocrinology, post residency, OR completed an accredited Fellowship program in Endocrinology or Obesity Medicine in lieu of years of experience • Active and unrestricted Medical State License in any US State, with eligibility to obtain additional state licenses • Currently opted-in with Medicare • Competency managing complex co-morbidities, including CKD, ESRD, and CAD/CV conditions • Expertise in advanced diabetes management, including CGMs, insulin pumps, and complex insulin regimens • Experience working with APPs, medical residents, or trainees in a clinical academic setting • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity • Open to working one of the following W-2 schedules: 32, 36, or 40 hours per week • Experience providing synchronous patient care in a telehealth setting is preferred • Bilingual medical fluency in Spanish is desired, but not required
