📋 Description

• Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management • Titrate insulin and use advanced diabetes technology, including CGMs and pumps • Collaborate with Endocrinologist colleagues and Registered Dietitians to co-manage a shared patient panel • Review charts and conduct return visits to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions • Provide advanced treatment planning and medical management support for complex endocrinology cases • Prescribe non-controlled medications for obesity and diabetes • Conduct weekly collaborative meetings and asynchronous communication with collaborating physicians • Ensure clinical documentation, protocols, and treatment pathways are followed • Support colleagues' professional development and continuing education • Participate in team-based care initiatives, departmental meetings, and quality improvement efforts

🎯 Requirements

• At least 2 years practicing advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens • Able to sign a collaboration agreement with an Endocrinologist • Active and unrestricted medical license; multi-state licenses are a plus • OMA certification (CAEOM) or demonstrated clinical experience in obesity medicine preferred • Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions • Experience delivering telemedicine care • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity • Committed to delivering patient-centered, culturally competent care
