📋 Description

• Treat patients via tele-health visits, including telephonic triage calls, video visit appointments, and email follow-ups • Participate in continuous learning during weekly Clinical Rounds and other modalities • Collaborate with virtual and in-office teammates via daily huddles • Apply specific clinical training to provide exceptional virtual patient care • Provide virtual clinical care 24 hours a day, 365 days a year as part of the Virtual Medical Team

🎯 Requirements

• Completed an accredited FNP program with a national certification • In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+) • Currently licensed in CA, OR, WA, AZ, or CO, with ability to obtain additional state licenses • For CA: 103 or 104 certification is strongly preferred, with willingness to obtain required • Ability to work weekday and weekend shifts; every other Saturday and/or Sunday required • Ability to work current shifts ranging from 6 am - 5 pm PT, 7 am - 6 pm PT, 11 am - 10 pm PT, 12 pm - 11 pm PT, or 1 pm - 12 am PT • Excellent clinical and communication skills • Passion for human-centered primary care • Ability to communicate with and provide care to individuals of all backgrounds • Ability to effectively use technology to deliver high quality care • Clinical proficiency in evidence-based primary care • Desire to be an integral part of a team dedicated to changing healthcare delivery • Openness to feedback and reflection • Ability to confidently navigate uncertain situations with patients and colleagues • Readiness to adapt personal and interpersonal behavior to meet patient needs • Authorization to work in the U.S. required
