Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Provide direct patient care focusing on evidence-based obesity treatment and chronic weight management • Review charts and conduct patient visits to ensure quality and continuity of care • Provide treatment planning and medical management support for complex cases or escalated concerns • Manage a patient panel and/or collaborate with APPs and Registered Dietitians specializing in obesity treatment • Collaborate with and supervise APPs as needed • Prescribe non-controlled obesity medications • Ensure clinical documentation, protocols, and treatment pathways are followed consistently • Participate in team-based care initiatives • Attend relevant departmental meetings • Contribute to quality improvement efforts within the obesity medicine program

🎯 Requirements

• Specialized in Obesity Medicine • Valid medical license to practice and prescribe legally in any US state • Eligibility to obtain additional state licenses as needed • American Board of Obesity Medicine (ABOM) certification • MD or DO degree • Valid and current Board Certification in Obesity Medicine by ABOM • Demonstrated clinical experience in obesity medicine and pharmacotherapy, including older generation oral and newer injectable medications • Active and unrestricted Medical State License in any US State • Minimum of 2 years as an attending physician, post-residency, in Internal Medicine, Family Medicine or Obesity Medicine, OR completion of an accredited Fellowship program in Obesity Medicine in lieu of experience • Interest in mentoring, teaching, and collaborating with APPs and Registered Dietitians specializing in obesity medicine • Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity • Must be currently opted-in with Medicare • Commitment to patient-centered, culturally competent care • Excellent communication and interpersonal skills • Open to working one of the following W2 roles: 32, 36, or 40 hours per week • Synchronous telehealth experience preferred • Medical fluency in Spanish desired but not required
