📋 Description

• Respond to inbound calls and inquiries about the care and wellness program • Research prior claim activity, questions, and spousal claim activity • Make outbound calls to policyholders and family members to educate them and encourage program participation • Use standardized tools to comprehensively assess policyholders and/or family members • Review recommendations and influence policyholders and family members to take action • Set up subsequent program steps with team colleagues based on policyholder needs • Review claim intake calls and prior claim information to understand current situations • Use clinical judgment to determine intervention suitability and need for additional care • Assess and address biological, psychological, environmental, and social needs • Use active listening and motivational interviewing during telephonic assessments • Synthesize assessment results and recommend appropriate program interventions • Build empathetic, professional relationships with policyholders and their care teams • Provide education on aging resources, medication management, daily living aids, and common medical conditions • Research resources, advocate for insureds, and provide preliminary caregiver coaching • Resolve routine policyholder and family questions using internal resources and escalation paths • Manage and prioritize a work queue and multiple responsibilities • Work independently and in an interdisciplinary team environment • Provide process improvement feedback and coach peers as appropriate

🎯 Requirements

• Passion for working with older adults and their family members • 2–5 years of professional experience with older adults and their families • College degree or certification/license in a care-management-related field, or equivalent work experience • Experience recruiting, selling, or enrolling seniors in programs • Experience performing assessments for older adults and caregivers • Superior written and verbal communication and customer service skills • Ability to meet weekly productivity standards while maintaining high quality • Ability to motivate individuals with post-operative musculoskeletal, neurological, and general acute problems • Experience making sound business decisions using risk management and quality protocols • Ability to work effectively independently and in a team environment • Strong relationship-building effectiveness with internal and external customers • Desire to learn Long Term Care Insurance contracts and benefits • Understanding of medical conditions common in aging and their effects on physical/cognitive function, prognosis, and rehabilitation • Working knowledge of Word, Excel, and PowerPoint • Willingness to learn claims and workflow systems • No employment visa sponsorship available
