📋 Description

• Conduct telehealth clinical evaluations to assess acute and chronic health needs, preventive care needs, and social drivers of health • Determine clinical appropriateness for Community Health Worker services and make referrals for care coordination and community-based support • Develop individualized care plans addressing clinical needs and social determinants of health factors • Collaborate with Community Health Workers to support patient engagement and follow-through • Address acute, chronic, preventive, and medication-management needs within program scope • Prescribe and renew non-controlled medications when clinically indicated • Document visits accurately and timely according to clinical, state, federal, and program requirements • Help patients navigate healthcare systems and connect to medical and community resources • Select shifts based on availability and patient demand during the PRN phase • Build toward a consistent schedule or full-time role as patient volume scales

🎯 Requirements

• Active, unrestricted Nurse Practitioner/APRN license and all applicable prescribing or furnishing authority required for practice in the state(s) in which the NP will provide care • Must be licensed in at least one of: Alaska, Arizona, California, Louisiana, New Mexico, or Nevada • Licensure in multiple states strongly preferred, with willingness to obtain additional state licenses as needed • Experience in primary care, family medicine, internal medicine, urgent care, or chronic disease management strongly preferred • Telehealth experience is a plus • Experience working with Medicaid and Medicare populations • Strong understanding of social determinants of health and their impact on patient outcomes • Comfortable with telehealth platforms and building rapport virtually • Excellent communication skills and ability to engage patients from diverse backgrounds • Familiarity with state, federal, and payer requirements applicable to CHW services and referrals, or willingness to learn • Spanish or other language fluency strongly preferred • No guaranteed minimum number of hours per week during the PRN phase
