📋 Description

• Manage Journey patient census and referrals for the assigned area • Review patient charts to determine eligibility and appropriateness for the Hospice benefit • Make initial phone calls to new Journey referrals and conduct follow-up calls • Schedule and conduct home visits with patients, families, and caregivers • Discuss end-of-life planning, Hospice philosophy, goals, eligibility, and benefits • Maintain patient charts and document visits and contacts • Build effective relationships and communication with Clinicians • Communicate and collaborate with the Palliative team and NP • Attend monthly Interdisciplinary team meetings • Meet weekly patient visit and phone call expectations and monthly hospice transition goals • Assist Hospice and/or Home Health teams with transitions to Hospice, including visits, calls, paperwork, and other tasks • Perform other duties as assigned

🎯 Requirements

• High school diploma/GED • Prior home healthcare or relative work experience in the healthcare industry preferred • Understanding of the hospice philosophy preferred • Must have and maintain in good standing professional license, certificate, or registration, as applicable • Successful completion of a drug screen prior to employment • Legally authorized to work in the United States • Ability to perform the essential duties and meet the stated physical demands, including frequently lifting and/or moving up to 20 pounds • Willingness and ability to travel by automobile and work in changing weather conditions
