📋 Description

• Work with administration, Care Managers, patients, and family members to reach healthcare goals and maintain communication • Support non-clinical duties and provide additional support to clinicians, case managers, and members related to patient care • Serve as a resource contact and education source to patients, families, providers, and staff • Provide telephonic follow-up with members, including post-hospital discharge and chronic condition management • Complete assigned Care Coordination tasks • Generate tasks for the assigned RN Case Manager • Direct patients and families to appropriate resources, including community resources • Assist, under RN Case Manager direction and supervision, with member goals, education, resource access, and follow-up care arrangements • Maintain quality service according to organizational standards • Assist with data entry, faxes, filing, and other clerical duties • Handle inbound and outbound calls and obtain information as requested by licensed staff

🎯 Requirements

• Ability to work 9a-6p CST or 10a-7p CST shift required • 1+ years related work experience in a healthcare related role • High school diploma or GED • Strong organizational and time management skills • Excellent verbal/written communication • Ability to communicate telephonically in a professional and efficient manner • Computer literacy in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint) • Ability to multi-task in a fast-paced environment
