📋 Description

• Support Humana's Illinois HealthChoice Medicaid Managed Care program • Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings • Assess members' unique care needs through telephonic and face-to-face assessments • Ensure members receive services in the least restrictive setting • Identify and resolve barriers to effective care • Develop and implement personalized care-plan interventions • Coordinate services and monitor and evaluate case management plans against members' personal goals • Continuously monitor member care and progress toward desired outcomes • Address immediate and acute gaps in care and access • Assist enrollees with filing grievances and appeals • Explain MCO requirements, services, and benefits • Provide information and referrals to community resources • Guide members and families toward appropriate care resources • Facilitate interactions with payers, providers, interdisciplinary teams, and others involved in members' care • Support either MLTSS or LTSS based on business needs • Travel 75–90% locally in the field to visit members • Report to the Manager, Care Management

🎯 Requirements

• Bachelor's degree in social sciences, social work, human services, or a related field, OR unrestricted Licensed Practical Nurse (LPN) in Illinois with one year of experience conducting comprehensive assessments and providing formal services for the elderly, OR Licensed Professional Counselor (LPC), OR Licensed Social Worker (LSW) • Must reside in Illinois • 1+ years of experience in health care and/or case management • 1+ years of experience with Medicare and Medicaid recipients and/or long-term care and/or Home and Community Based Services and/or managed care organizations • Intermediate to advanced computer skills • Experience with Microsoft Word, Excel, and Outlook • Experience using electronic information applications/software programs, including electronic medical records • Valid state driver's license • Personal vehicle liability insurance meeting required limits • Required TB screening for selected candidates • Ability to work Monday–Friday, 8am–5pm CST, with flexibility based on business needs • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Home internet with at least 25 Mbps download and 10 Mbps upload speed
