📋 Description

• Enroll patients in chronic care management and/or remote patient monitoring programs • Partner with cardiology, internal, primary, and family medicine physician groups and healthcare systems • Reach out to identified patients and enroll them in Care Management Programs • Explain program benefits, patient financial responsibility, consent requirements, device shipment, and program enrollment • Ensure a seamless, complete, and effective enrollment process • Serve as the initial point of contact for pending enrollee calls • Manage a high open caseload until enrollment is complete • Provide exceptional customer service and a positive experience for prospective patients and caregivers • Navigate multiple systems and portals to input patient enrollment information • Achieve monthly new enrollee goals • Perform additional duties as assigned

🎯 Requirements

• Experience in a high-volume inbound and outbound call environment required • Enrollment/Medical Sales experience preferred • Experience in patient enrollment and call center experience • Poise and patience when dealing with patients • Comfortable communicating with patients over the phone • Ability to overcome objections • Good problem-solving skills • Keen attention to detail • Metrics-driven • Excellent time management skills • High computer literacy • Engaging personality and desire to connect with patients • High empathy and collaborative teamwork • Flexibility and openness as the role and responsibilities may change and/or evolve • Ability to make outbound calls and provide information about remote patient monitoring and chronic care management programs
