📋 Description

• Conduct thorough cardiovascular patient assessments through telehealth visits • Formulate differential diagnoses and develop treatment plans • Prescribe medications and order diagnostic tests • Manage chronic cardiovascular conditions remotely, especially advanced Heart Failure, hypertension, arrhythmias, and coronary artery disease • Monitor patient progress, analyze real-time data, and adjust care pathways to optimize cardiac function and prevent hospital readmissions • Implement, titrate, and manage Guideline-Directed Medical Therapy (GDMT) according to ACC/AHA guidelines • Educate patients and families about cardiovascular disease, prevention, lifestyle modifications, medication adherence, fluid and diet management, exercise, and smoking cessation • Collaborate with cardiologists, primary care providers, and other healthcare professionals • Participate in multidisciplinary care teams, research studies, and quality improvement initiatives • Maintain accurate patient records, assessments, treatment plans, progress notes, and discharge summaries • Advocate for patients' needs and preferences • Pursue continuing cardiology education and maintain relevant certifications • Participate in quality assurance programs

🎯 Requirements

• Master of Science in Nursing (MSN) with a specialization in cardiology or a related advanced practice nursing field • 3+ years of dedicated experience as a Nurse Practitioner in an outpatient cardiology setting or clinic environment • Active, current, and unrestricted Nurse Practitioner (NP) license, preferably in a Compact State • Nurse Practitioner certification specific to cardiology from a recognized certifying body, such as ANCC or AANPCB, in a relevant NP specialty • ACLS certification is required or highly recommended • Proficiency in outpatient management and interpretation of complex diagnostic tests, including ECGs and echocardiograms • Expertise in optimizing clinical outcomes for advanced Heart Failure populations • Proven expertise in applying, optimizing, and titrating Guideline-Directed Medical Therapy (GDMT) • Excellent communication skills for virtual patient education and multidisciplinary collaboration • Ability to make critical decisions and adapt treatment plans based on patient responses and changing clinical conditions • Familiarity with EHR systems, remote patient monitoring (RPM) tools, and digital healthcare technologies • Must be able to work Monday through Friday, 9 am to 5 pm CST
