📋 Description

• Conduct thorough cardiovascular patient assessments via telehealth, including medical history reviews, physical examinations, and diagnostic tests • Formulate differential diagnoses and comprehensive treatment plans • Prescribe medications and order diagnostic tests • Manage chronic cardiovascular conditions remotely, emphasizing advanced heart failure, hypertension, arrhythmias, and coronary artery disease • Monitor patient progress, analyze real-time data, and adjust care pathways to optimize cardiac function and prevent hospital readmissions • Implement, titrate, and manage Guideline-Directed Medical Therapy (GDMT) according to ACC/AHA guidelines • Educate patients and families about cardiovascular disease, prevention, lifestyle modifications, medication adherence, fluid and diet management, exercise, and smoking cessation • Collaborate with cardiologists, primary care providers, and other healthcare professionals • Discuss complex cases and participate in multidisciplinary care teams • Stay current with cardiology research and incorporate evidence-based practices • Participate in research studies or quality improvement initiatives • Maintain accurate patient records, assessments, treatment plans, progress notes, and discharge summaries • Advocate for patients’ needs and preferences • Pursue continuing education and maintain relevant certifications • Participate in quality assurance programs • Work Monday through Friday, 9 am to 5 pm PST

🎯 Requirements

• Master of Science in Nursing (MSN) with a specialization in cardiology or a related advanced practice nursing field • 3+ years of dedicated experience working as a Nurse Practitioner in an outpatient cardiology setting or clinic environment • Must hold an active, current, and unrestricted Nurse Practitioner (NP) license, preferably in a Compact State • Obtain and maintain Nurse Practitioner (NP) certification specific to cardiology from a recognized certifying body, such as ANCC or AANPCB, in a relevant Nurse Practitioner specialty • Certification in ACLS is required or highly recommended • Demonstrated proficiency in outpatient management • Ability to interpret complex diagnostic tests, including ECGs and echocardiograms • Expertise in optimizing clinical outcomes for advanced Heart Failure populations • Proven expertise in practical application, optimization, and protocol titration of Guideline-Directed Medical Therapy (GDMT) • Excellent communication skills for virtual patient education and multidisciplinary collaboration • Ability to make critical decisions, analyze complex cardiac cases, and rapidly adapt treatment plans • Compassion and empathy when working with patients and families • Meticulous attention to detail in patient assessments, virtual documentation, and treatment plans • Familiarity with electronic health records (EHR) systems, remote patient monitoring (RPM) tools, and digital healthcare technologies • Must be able to work Monday through Friday, 9 am to 5 pm PST
