📋 Description

• Provide high-quality patient care to acute care patient populations in the home • Use monitoring, assessment, planning, intervention, and critical-thinking skills • Navigate patients and families across Hicuity Health’s phases of care • Monitor and respond to biometric data according to Hicuity Health protocols • Conduct home and audio/visual patient-care visits as directed by the care team or protocol • Implement care-plan interventions efficiently using a patient-centered approach • Use Hicuity Health team and supplier-partner resources to support collaborative workflows, standards, policies, protocols, guidelines, and documentation systems • Follow clinical protocols and complete necessary training for evidence-based care • Promote professional practice and a culture of safety • Participate in process-improvement efforts • Obtain certifications required by Hicuity Health

🎯 Requirements

• Part-time schedule: 7:00 p.m.–11:30 p.m. PST, minimum 3 shifts per week • Must have an active, unencumbered California state license • RN Compact (multi-state) license in addition to a California license preferred • Three years of RN experience • Med/Surg or nursing experience in an inpatient hospitalized setting required • Clinical knowledge • Ability to promote patient safety and a positive patient experience • Flexibility and comfort with a changing, dynamic work environment and schedule across all shifts and days • Ability to work independently and as part of a team • Ongoing learning and continuing education to care for varied patient populations • Ability to collaborate, advocate, and mentor others professionally • Collaborative, respectful relationships with colleagues and customers • Must obtain certifications as required by Hicuity Health • This job may be performed remotely from anywhere in the United States, except it may not be filled or performed in Washington, California, New York, Nevada, Oregon, Pennsylvania, Montana, Illinois, or Colorado
