📋 Description

• Provide in-home health evaluations in members’ private residences • Engage with members and build trusted relationships • Review medications and medical histories • Perform physical evaluations • Provide additional diagnostic and preventive services, including diabetic eye exams, spirometry, and bone density screening • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Help identify connections to appropriate care and bridge gaps in care • Use an iPad and intuitive clinical workflow for each evaluation • Avoid prescribing medication, ordering lab tests, or altering current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Nurse Practitioner or Physician Assistant qualification • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to perform medication and medical history reviews, physical evaluations, and diagnostic tests as indicated
