📋 Description

• Manage a portfolio and caseload of low- to moderate-acuity clients navigating the healthcare system • Build trusted relationships and guide clients throughout their healthcare journey • Provide education, advocacy, care coordination, and ongoing clinical support • Conduct clinical intakes by email or phone to assess needs and determine next steps • Help clients access appropriate providers, facilities, and services • Coordinate appointments and assist with medical record review and support • Provide provider recommendations and clinical guidance • Ensure smooth transitions and communication across clinical and operational teams • Welcome, educate, and triage new inbound clients • Support the Triage Team with inbound client needs and service inquiries • Communicate with clients, caregivers, providers, and internal partners

🎯 Requirements

• Active Registered Nurse license in the state of residence • At least five years of healthcare experience • Experience in case management, care coordination, patient navigation, ER Triage, or a related broad-based clinical environment • Confidence, empathy, critical thinking skills, and sound clinical judgment • Self-directed, collaborative approach • Strong organization and time management skills • Comfort learning new technologies and systems • Experience working remotely • Strong written and verbal communication skills • Ability to translate complex clinical information for clinical and non-clinical audiences • Must live in and physically perform all work in the United States
