📋 Description

• Manage members' health issues using telephonic strategies, approaches and techniques • Identify and resolve barriers that hinder effective care • Conduct comprehensive assessments and continuously monitor patient care and progress toward desired outcomes • Develop and use active care plans based on assessments, data and conversations with members • Guide members and their families to essential healthcare and community resources • Follow professional concepts, standards, regulations, strategies, operating standards and established procedures • Make decisions regarding work approach and priorities under managerial direction • Report to the Manager, Care Management

🎯 Requirements

• Bachelor's degree in a health-related field, active Illinois LPN licensure, or ability to obtain an Illinois LPN license • 2 or more years of case management, medical case management, care coordination, or healthcare support experience • Comprehensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook • Bilingual English/Spanish preferred • Certified Case Manager (CCM) preferred • Master's Degree preferred • Managed care experience preferred • Prior experience with Medicare and Medicaid recipients preferred • Experience with electronic case note documentation and multiple computer applications/systems preferred • Experience with health promotion, coaching and wellness preferred • Knowledge of community health and social service agencies and community resources preferred
