📋 Description

• Assess and evaluate Medicaid members’ behavioral health needs and requirements • Perform face-to-face and telephonic assessments • Serve as the primary point of contact and provide integrated care coordination • Use evidence-based behavioral health services, personalized care coordination, crisis intervention, and peer support • Support members with SMI, SED, SUD, and justice-involved populations • Engage members in their communities and conduct face-to-face visits • Complete required assessments, including the Comprehensive Risk Assessment (HRA) • Coordinate behavioral health and medical services and ensure provider engagement and treatment-plan adherence • Improve health literacy and address health-related social needs • Serve as the member’s interdisciplinary care team, overseeing care planning, transitions, and service delivery • Facilitate interdisciplinary care team meetings and communication among providers, Service Coordinators, and Care Management Extenders • Support transitions of care • Conduct biannual and quarterly face-to-face visits • Work autonomously and seek support when needed • Collaborate with internal departments, providers, and community-based organizations • Follow processes and procedures to comply with IDHS, CMS, and NCQA requirements • Travel up to 25% for collaboration, face-to-face meetings, and field interactions • Perform other duties as assigned

🎯 Requirements

• Active Illinois licensed LCSW, LMFT, or LCPC; no provisional licenses • Must reside in Illinois or a border state • Minimum of 2 years of post-degree clinical experience in a behavioral health setting • Case management experience working with complex SMI, SUD, and SED populations • Ability to travel to region-based facilities and homes for face-to-face assessments • Required TB screening if selected • Valid state driver’s license • Proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits • Case Management Certification (CCM) preferred • 3+ years of care coordination experience, including in-home assessments preferred • Experience working with Medicaid populations preferred • Field case management experience preferred • Knowledge of community health and social service agencies and additional community resources preferred • Previous managed care experience preferred • Bilingual preferred • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Must work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
