📋 Description

• Assess and evaluate members' needs in a telephonic environment • Guide members and families toward resources appropriate for their care and wellbeing • Manage members' physical, environmental, and psychosocial health issues • Resolve barriers that hinder care • Monitor patient care through assessments and evaluations to ensure progress toward desired outcomes • Create member care plans as needed • Support the Transition of Care (TOC) program and continuity of care between settings • Assist with HEDIS completion follow-up visits after facility discharge for TOC • Conduct field visits based on member needs and program requirements • Collaborate with staff, providers, members, and families through occasional travel and face-to-face meetings • Report to the Manager, Care Management

🎯 Requirements

• Must live in the state of South Carolina • Must have an active South Carolina license as an LPC, LISW-CP, or LISW-AP • 1 or more years of related work experience • Bachelor's or master's degree in a behavioral-health field preferred • 1 year of managed care experience preferred • Transition of care (TOC) experience for Physical Health and/or Behavioral Health preferred • 1 year of Inpatient Behavioral Health or Case Management experience in a hospital setting preferred • CCM certification preferred • Experience working with the Medicaid population preferred • Knowledge of community health and social service agencies and additional community resources preferred • Minimum download speed of 25 Mbps and upload speed of 10 Mbps • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
