📋 Description
• Provide comprehensive care management for Disease Management programs • Interact with beneficiaries to address clinical conditions • Coordinate care with healthcare providers • Collaborate within the Care Management programs
🎯 Requirements
• U.S. Citizenship • Successfully receive interim approval for government security clearance • Active RN license in state of employment • 3 years of clinical or managed care RN experience • Current CCM certification • Bachelor's degree in nursing (preferred) • Experience with Case/Disease management programs (preferred) • Certified Diabetes or Asthma Educator (preferred) • TRICARE experience (preferred)
