📋 Description

• Assess and evaluate Medicaid members' needs and requirements • Visit members in homes, assisted living facilities, long-term care facilities, and other care settings • Coordinate services and resources to support members' optimal wellness • Establish member-specific care plans • Ensure members receive services in the least restrictive setting • Plan and implement interventions • Monitor and evaluate case management plans against members' personal goals • Guide members and families toward appropriate care resources • Facilitate interaction with payer sources, providers, interdisciplinary teams, and others involved in members' care • Identify and resolve barriers to effective care • Continuously monitor patient care through assessments, data, conversations, and active care planning • Travel locally 75–90% of the time • Report to the Regional Care Coach Manager

🎯 Requirements

• Must live in Collier county in Florida • Bachelor's degree in health and human services field • 2+ years of related experience • Exceptional verbal/written skills • Use of a variety of electronic information applications/software programs • Valid state driver's license • Reliable vehicle • Proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits • Must meet Florida Level 2 background screening requirements through the Care Provider Background Screening Clearinghouse • 25 Mbps minimum download speed and 10 Mbps minimum upload speed for home internet • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
