📋 Description

• Visit Medicaid members in their homes, Assisted Living Facilities, Long Term Care Facilities, and other care settings • Conduct 75-90% local travel • Assess and evaluate member needs and requirements • Establish member-specific care plans • Ensure members receive services in the least restrictive setting • Plan and implement interventions • Coordinate services • Monitor and evaluate case management plans against members' personal goals • Guide members and families toward appropriate care resources • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in member care • Report to the Regional Care Coach Manager • Educate and encourage members regarding their health • Connect members with quality services to promote well-being and health outcomes

🎯 Requirements

• Bachelor's Degree with at least 2 years of relevant experience in Health Services, Social Work, or Psychology; OR Registered Nurse (RN) licensed in Florida with at least 2 years of relevant experience; OR Licensed Practical Nurse (LPN) licensed in Florida with at least 4 years of relevant experience; OR Case Worker with a minimum of 6 years of Health Services/Case Management Experience • Prior experience with Medicare & Medicaid recipients • AHCA Fingerprint Required • Required tuberculosis screening • Valid state driver's license • Proof of personal vehicle liability insurance with 100,000/300,000/100,000 limits • Ability to work Monday-Friday, 8:30am-5:00pm • Minimum home internet speed of 25 Mbps download and 10 Mbps upload for home or hybrid home/office employees • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Prior nursing home diversion or long term care case management experience • Bilingual English/Spanish with ability to pass a Language Proficiency Exam • Experience with electronic case note documentation and multiple computer applications/systems • Experience working with geriatric population • Experience with health promotion, coaching and wellness • Knowledge of community health and social service agencies and community resources • 701B Certification • Medication application (RFA) experience
