📋 Description

• Visit Medicaid members in their homes, Assisted Living Facilities, Long Term Care Facilities, and other care settings • Travel locally 75–90% of the time • Assess and evaluate member needs and requirements • Establish member-specific care plans • Ensure members receive services in the least restrictive setting • Plan and implement interventions • Coordinate services and monitor and evaluate case management plans against members’ personal goals • Guide members and families toward appropriate care resources • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members’ care • Report to the Regional Care Coach Manager

🎯 Requirements

• Bachelor's degree in a health and human services field • 2 or more years of related experience • Knowledge of Microsoft Office Word and Excel • Exceptional verbal and written communication and interpersonal skills • Ability to use a variety of electronic information applications and software programs • Ability to pass tuberculosis (TB) screening • Passion for contributing to continuous improvement of consumer experiences • Valid state driver's license • Maintain personal vehicle liability insurance meeting state minimums or specified higher limits • Must reside in Hillsborough or Hardee counties in Florida • Must have a separate room with a locked door usable as a private home office • Previous electronic case note documentation experience preferred • Prior Medicare and Medicaid recipient experience preferred • Health promotion, coaching, and wellness experience preferred • Knowledge of community health and social service agencies and community resources preferred
