📋 Description

• Assessing and evaluating member's needs and requirements • Visiting Medicaid members in their homes and other care settings • Establishing member specific care plans • Ensuring members receive services in the least restrictive setting • Planning and implementing interventions to meet members' needs • Coordinating services and monitoring case management plans • Guiding members towards appropriate care resources

🎯 Requirements

• Bachelor's degree in health and human services field • 2 or more years of related experience • Knowledge of Microsoft Office Word and Excel • Exceptional verbal/written communication and interpersonal skills • Ability to use a variety of electronic information applications/software programs • Valid state driver's license and personal vehicle liability insurance • Must have a separate room with a locked door that can be used as a home office for privacy
