📋 Description

• Supervise and oversee the Care Coordinator SNP team • Support health care management and coordination of care for SNP members with complex and chronic care needs • Monitor timely completion of Health Risk Assessments, Individualized Care Plans, and Interdisciplinary Care Team delivery • Provide guidance to staff and manage complicated coordination requests from members, providers, or staff • Assist with development and oversight of SNP Care Coordination programs, projects, workflows, SOPs, and reports • Collaborate with supervisors and managers to improve processes and workflow • Serve as a resource and liaison for members, providers, community partners, management, and the care coordination team • Review program-performance reports, identify gaps, and support compliance with internal goals and regulatory requirements • Participate in SNP quality audits, including CMS and vendor audits, report results, and identify process improvements • Assign time-sensitive tasks to Care Coordinators • Mentor, train, audit, coach, and manage non-clinical Care Coordinator staff • Direct administrative work including maintaining member records, correspondence, case openings, and task creation in the Care Management platform • Assist with Case Management team coverage plans • Maintain confidentiality of information among health care professionals • Recruit, select, orient, train, assign workload, monitor, appraise, coach, counsel, and discipline assigned employees • Perform other duties as assigned

🎯 Requirements

• 3+ years of Care Coordination experience • 1+ year of supervisory or lead experience in a health care setting • High school diploma or GED, with one to three months related experience and/or training, or equivalent combination of education and experience • Ability to analyze data for trends • Knowledge of Managed Care Plans and medical terminology • Computer skills, including Microsoft Outlook, Excel, and Word • Typing speed of 25 WPM • Good oral, written, and telephone skills • Ability to apply common-sense understanding to detailed written or oral instructions • Ability to deal with problems involving a few concrete variables in standardized situations • Good organizational skills • Ability to reason and carry out instructions • Good interpersonal skills • Fluency in spoken and written English • Ability to file systematically • Ability to perform essential physical functions, including standing, walking, sitting, using hands, reaching, talking, and hearing • Ability to frequently lift and/or move up to 10 pounds • No licensure required
