📋 Description

• Assess and evaluate Medicaid members' needs and requirements • Establish member-specific care plans and coordinate services • Ensure members receive services in the least restrictive setting • Plan and implement interventions to meet care needs • Monitor and evaluate case management plans against members' personal goals • Guide members and families toward appropriate care resources • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members' care • Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings • Conduct face-to-face assessments involving 75–90% local travel • Identify and resolve barriers to effective care • Continuously monitor patient care using assessments, data, conversations with members, and active care planning

🎯 Requirements

• Applicants must reside in Cook or Will County, Illinois, within one of the listed ZIP codes or within a 10-mile radius • Active Illinois license in LCSW, LMFT or LCPC; no supervisees or provisional licenses • 2+ years of post-degree clinical experience in a behavioral health setting • Case management experience working with complex SMI, SUD, and SED populations • Ability to travel to region-based facilities and homes for face-to-face assessments • Ability to use electronic information applications/software programs, including electronic medical records • Intermediate to Advanced computer skills • Experience with Microsoft Word, Outlook, and Excel • Valid driver's license, car insurance, and reliable transportation • Must complete TB screening if selected • Must maintain personal vehicle liability insurance meeting the stated limits • Home internet with at least 25 Mbps download and 10 Mbps upload speed • Dedicated work area without ongoing interruptions to protect PHI/HIPAA information
