📋 Description

• Evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Manage members' physical, environmental and psychosocial health issues • Resolve barriers that hinder care • Conduct assessments and evaluations to ensure patients progress toward desired outcomes • Create member care plans as needed • Understand department, segment and organizational strategy and operating goals • Report to the Manager, Care Management Behavioral Health • Perform mandatory field work and travel up to 50% of the time

🎯 Requirements

• Bachelor's degree in health and human services field • 2 or more years of related experience • Knowledge of Microsoft Office Word and Excel • Exceptional verbal/written communication and interpersonal skills • Ability to use a variety of electronic information applications/software programs • Required TB screening for this patient-facing role • Valid state driver's license • Personal vehicle liability insurance meeting minimum requirements • Separate room with a locked door for use as a private home office • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Preferred: experience with electronic case note documentation and multiple computer applications/systems • Preferred: experience with Medicare & Medicaid recipients • Preferred: experience with health promotion, coaching and wellness • Preferred: knowledge of community health and social service agencies and community resources
