📋 Description

• Assess and evaluate members' needs and requirements • Meet members in their locations and assess needs and barriers • Connect members and families with appropriate care, wellbeing, and community resources • Administer ongoing long-term services and support (LTSS) assessments using person-centered approaches • Contact members telephonically and/or in person to establish goals and priorities • Evaluate resources and develop plans of care • Identify LTSS providers and community partnerships • Develop and modify service plans with applicable care team members • Support members with LTSS, environmental, social, housing, educational, and other service navigation • Use available member information to avoid duplicative assessments • Assist members in maintaining Medicaid eligibility • Collaborate with Medical Director, Geriatrician, and Care Coordinator to ensure cohesive service delivery and positive outcomes

🎯 Requirements

• Bachelor's degree in health and human services field • 2 or more years of related experience • Knowledge of Microsoft Office Word and Excel • Exceptional verbal/written communication and interpersonal skills • Ability to use a variety of electronic information applications/software programs • Must complete tuberculosis (TB) screening if selected • Valid state driver's license • Personal vehicle liability insurance meeting state minimum limits or specified Humana limits, whichever is higher • Must have a separate room with a locked door usable as a home office • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Licensed RN or Licensed Master's Social Worker (preferred) • Prior experience with Medicare and Medicaid recipients (preferred) • Experience with electronic case note documentation and multiple computer applications/systems (preferred) • Experience with health promotion, coaching, and wellness (preferred) • Knowledge of community health and social service agencies and community resources (preferred)
