📋 Description

• Serve as the primary point of contact for Care Coaches • Maintain team compliance • Collaborate with other departments • Support a team of 15–17 Care Coaches • Manage coaching and accountability models • Support metric-driven reports • Identify barriers and supports to overcome them • Maintain caseload ratios while minimizing mileage and overtime • Monitor patient care through assessments, data, conversations with members, and active care planning • Ensure patients progress toward desired outcomes • Make decisions regarding resources, approaches, and tactical operations for departmental projects and initiatives • Conduct cross-departmental briefings and area meetings • Maintain frequent contact with other department managers • Report to the Regional Care Coach AD

🎯 Requirements

• Bachelor's Degree in a health-related field • 2 or more years of managed care experience in Medicare and/or Medicaid • 1 or more years of management/leadership experience • 1 or more years of complex membership experience • Proficiency in analyzing and interpreting trends • Progressive experience in the health solutions industry, with emphasis with leading and managing teams • Valid state driver's license • Proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits • Must reside within 3 hours of Broward County • Minimum home internet download speed of 25 Mbps and upload speed of 10 Mbps • Dedicated work space without ongoing interruptions to protect member PHI/HIPAA information • RN or LPN with current State Licensure • Master's degree in human services, social sciences, or health related field • Applicable State licensure in field of study • Proficiency in analyzing and interpreting financial trends • Previous experience in utilization management, discharge planning and/or home health or rehabilitation • Experience with health promotion, coaching and wellness • Health Plan experience • Previous Medicare/Medicaid experience • Experience maintaining NCQA standards • Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse
