📋 Description

• Build, lead, and inspire the team of Social Worker Case Managers and behavioral health care coordinators • Serve on TOC's interdisciplinary management team alongside physician, NP/PA, RN case management, and care coordination managers • Build and develop a high-performing team with a culture of service focused on patient outcomes • Track staff performance through competency check-offs, audits, shadowing, and 1:1s, and provide coaching • Create workflows, assessments, and plans of care to improve team efficiency and measurable patient results • Support the team on escalated patient needs • Lead onboarding for new teammates and ongoing education for existing staff • Partner with cross-functional teams to drive execution and standardize workflows across programs • Meet regularly with the Senior Manager of the RN and SW TOC functions to review team performance, wins, and areas for improvement • Spend 30–40% of time on direct patient outreach and case work

🎯 Requirements

• An active social work license (LMSW or higher) • 3+ years of experience in case management • 3+ years of experience in people management, including developing and retaining a high-performing team • Experience working with — and preferably managing — interdisciplinary care management teams • Ability to independently manage a fully remote schedule, M–F, 8:30am–5:00pm CST, MST, or PST • Experience leading clinical teams and operations with direct accountability for health outcomes, including driving new initiatives to measurable results • Strong analytical instincts paired with strong clinical social work skills • Ability to set quantitative goals and act on data • Confidence training a team to manage complex patients so needs don't fall through the cracks
