📋 Description

• Assess members’ health and safety needs to develop comprehensive Member Care Plans • Conduct regular health assessments • Coordinate with acute and primary care providers • Educate members on disease processes, risks, nutrition, exercise, and lifestyle modifications • Document in member files within required DHS contract timeframes • Participate in monthly member phone contacts and travel to in-person visits • Complete record reviews for reassigned members and identify remediation needs • Support record review and remediation activities • Complete remediation documentation • Inform newly assigned RNs and Care Coach partners of missing or needed follow-up • Assign tasks in the D365 system • Accept additional complex member assignments • Support new member assignments • Travel approximately 40% of the time to visit members • Provide care coordination, support, and education to Wisconsin Family Care Program members

🎯 Requirements

• Associate degree in nursing • Licensed Registered Nurse in the state of Wisconsin (in good standing) • Valid state driver's license • Proof of personal vehicle liability insurance with at least 100/300/100 limits • Ability to work Monday through Friday, 8:00 am–4:30 pm CST • Ability to work remotely from a dedicated space without ongoing interruptions • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Patient-facing role requiring TB screening if selected • Reside within 45 minutes or less of the assigned coverage area (preferred) • 3 years Care Management experience (preferred)
