📋 Description

• Coordinate program operations related to the Coordinated Entry System process and workflow • Match housing referrals to program openings and unit vacancies • Monitor CES workflows to ensure resource alignment • Support system navigation activities to reduce move-in timelines • Address system- and program-level barriers to ensure continuous CES outflow • Liaise with housing providers, outreach teams, shelters, access sites, and community partners • Implement CoC CES policies, procedures, assessment tools, prioritization policies, referral workflows, HMIS technologies, eligibility requirements, and matching protocols • Administer procedures to reduce unit vacancies, expedite referrals, and improve move-in processes • Gather and analyze household and program-level data to remove housing barriers and improve CES processes • Implement standardized CES guidance, housing applications, service coordination, and data tracking • Collaborate across agencies and systems to identify workflow issues and implement solutions • Develop training materials, compliance training, operational guides, FAQs, and toolkits • Translate policy requirements and technical documentation into action-oriented CES processes • Coach and facilitate CES partners to ensure fidelity to CES design standards • Identify process improvements to increase timely referrals to eligible and prioritized housing options • Provide field-based support, staffing augmentation, direct training, technical assistance, capacity building, and quality assurance monitoring as assigned • Develop working relationships with CES providers, access sites, affordable housing providers, behavioral health partners, and substance use treatment facilities • Examine HMIS and CES data to identify trends and inequities and recommend improvements • Maintain secure communications protecting client privacy and confidentiality

🎯 Requirements

• Bachelor’s degree in Human Services, Public Policy and Administration, Business, Social Welfare, or related fields; equivalent experience may be considered • Minimum two years of directly related experience with Continuum of Care programs and HUD Coordinated Entry programs • Understanding of Continuum of Care programs and HUD Coordinated Entry requirements, including the CoC Program interim rule and HUD Notice CPD-17-01 • Knowledge of HUD resources and guidebooks on Coordinated Entry System topics • Excellent verbal, interpersonal, and written communication skills • Ability to explain complex concepts to non-technical stakeholders • Strong organizational, facilitation, conflict resolution, and problem-solving skills • Ability to prioritize and manage multiple activities simultaneously while meeting stringent deadlines • Experience managing sensitive information and maintaining confidentiality • Knowledge of Housing First, trauma-informed care, person-centered service delivery, and racial equity principles • Experience facilitating case conferencing or multidisciplinary coordination meetings, managing conflict, and attaining consensus across multiple stakeholders
