📋 Description

• Work within the community providing outreach to patients with complex medical and social needs • Act as a resource to help patients navigate the health care system • Orient and assess new Community Health Workers • Regularly assess staff proficiency during orientation and support training progression or remediation • Identify and conduct outreach to medically complex patients • Collaborate with multidisciplinary teams to develop and implement care plans • Engage patients by phone, home visits, hospital visits, and accompanying patients to provider appointments as needed • Complete vital signs • Identify gaps in care and connect members with resources • Facilitate telemedicine visits with members and providers • Provide disease-specific education • Conduct medication reviews to support medication adherence • Conduct safety evaluations in patients' homes • Facilitate treatment-plan adherence through early identification and resolution of care gaps • Communicate concerns to the clinical team and manager and seek support and guidance proactively • Participate in multidisciplinary team conferences to maximize treatment plans • Reinforce treatment plans under the direction of the Case Manager and Licensed Social Worker • Provide consultation and case review as needed

🎯 Requirements

• High School Diploma or Equivalent (GED) required • Minimum of 1 year related work experience required • Certified Community Health Worker (CCHW) certification from the Pennsylvania Certification Board • Clerical skills • Communication skills • Multitasking • Problem-solving skills • Ability to work days • Ability to work 40 scheduled hours per week
