📋 Description

• Perform Licensed Practical Nurse duties as a patient navigator for incoming calls and new patient program enrollment • Provide patient education and promote understanding of health conditions • Provide ongoing continuity of care and clinical support to providers and patients • Use clinical knowledge to determine actions facilitating patient introduction to the program • Evaluate and document patient needs primarily by telephone or documentation review • Coordinate and implement patient plans of care, including evaluation, treatment, and surveillance • Communicate with patients by mail, telephone, and in person • Coordinate patient needs, activities, support groups, educational series, and available support services • Serve as point of contact for providers and liaison with Scheduling Services • Maintain timely patient follow-up and surveillance systems and processes • Develop and implement patient screening tools, clinic procedures, and navigational procedures • Investigate patient concerns involving appointment access, communication, and billing • Maintain clinical supply inventory and exam and treatment rooms • Conduct medical record reviews and report findings to physician specialists • Provide nursing care and assist physicians with procedures • Inspect equipment and identify preventive or corrective maintenance needs

🎯 Requirements

• Minimum of 2 years previous LPN work experience required • Graduate from Specialty Training Program-Nursing required • Licensed Practical Nurse license required • Proficient computer skills • Proficient with Microsoft Word, Excel, PowerPoint, SharePoint, Windows, and Teams • Clinic and Pediatrics experience preferred • Ability to work day/evening hours between 7:00 a.m. and 11:30 p.m. • Ability to work every other weekend • Ability to work at least one major and one minor holiday per year • Travel may be required for orientation/training, meetings, or other business needs (less than 25%) • Communication, computer literacy, customer service, multitasking, and teamwork skills
