📋 Description

• Review medical records from multiple EHRs and on-site hospital and professional inpatient or outpatient records • Identify principal diagnoses, secondary diagnoses, and procedures performed • Review laboratory findings, radiology reports, scan reports, discharge summaries, histories and physicals, consultations, orders, progress notes, and ancillary treatment records • Assign codes according to hospital and professional coding guidelines, Coding Clinic directives, federal regulations, CCI coding initiatives, CPT Assistant, and other standard guidelines • Provide material to the RACE team to support physician education and clarify documentation • Assist with physician coding education and HCC targeting decisions • Use encoder software and EPIC EMR expertise to support accurate coding • Develop and maintain expertise in non-EPIC EMR systems as appropriate • Update and correct historical file data by submitting delete files and notifying the RACE team • Ensure complete and accurate documentation, code assignment, and reporting for payer reimbursement • Maintain strict confidentiality of financial and medical records • Comply with job-specific obligations and organizational policies and procedures

🎯 Requirements

• High School Diploma or Equivalent (GED) (Required) • One relevant certification from AHIMA or AAPC required upon hire • Acceptable AHIMA certifications include CCS, CCS-P, RHIT, RHIA, and CCA • Candidates with only a CCA must obtain a CCS, RHIT, or RHIA within 12 months of hire • AAPC certifications are acceptable except Scribe, Documentation, Instructor, International Credentials, CPB, RCMS, CVBA, CPPM, and CPCO • Minimum of 1 year related work experience (Required) • Computer literacy • Critical thinking • Knowledge of hospital and professional coding guidelines, Coding Clinic directives, federal regulations, CCI coding initiatives, CPT Assistant, or other standard coding guidelines • Knowledge of anatomy and physiology, disease processes, medical terminology, diagnostic studies, treatment modalities, and disease procedures • Ability to maintain strict confidentiality of financial and medical records • Ability to work with multiple EHR systems, including EPIC • Relevant experience may combine related work experience and completed specialty training program, with 1 year of specialty training equaling 1 year of relevant experience
