📋 Description

• Serve as first responder for application-level production issues during US business hours, owning them from investigation through shipped fix • Share production coverage with the CTO and infrastructure engineer, with mutual backup and no solo always-on pager • Keep releases moving through branch syncs, release shepherding, and maintaining a healthy CI pipeline • Harden fast-shipped, often AI-assisted code with error handling, retries, edge-case handling, test coverage, and security review for PHI-related changes • Review, harden, and land agent-authored pull requests after verifying them against actual system behavior • Build observability through error reporting, synthetic canaries for critical journeys, and alerting • Turn recurring incidents into runbooks and permanent fixes • Refactor, simplify, and delete code to keep the system simple as it grows • Own schemas, indexes, and consistency across related data collections • Design and run safe migrations and backfills for inconsistent historical data • Add write-time guardrails to prevent recurring data problems • Productionize and own monitoring and alerting for the ADT (HL7v2) ingestion pipeline • Investigate hospital feed issues and trace problems from feed to patient record • Partner with the infrastructure engineer on the application and data side of ADT ingestion • Work directly with the CTO on a small, fast-moving team with broad ownership • Take features from design through production and grow into full feature ownership • Use Claude Code and AI agents to investigate production issues, verify findings, and ship fixes • In the first 6–12 months, improve observability, harden the application layer, reduce incidents, and run the ADT pipeline reliably in production

🎯 Requirements

• A seasoned application engineer (roughly 5+ years, or equivalent demonstrated judgment) with real production experience • Strong TypeScript across React (frontend) and Node (backend) • Comfortable in a GraphQL API and a document database; Apollo and MongoDB experience or comparable experience • Able to read schemas, reason about indexes and query performance, and write safe migrations or backfills • Comfortable debugging event-driven systems, including queues, retries, redelivery, and idempotency • Daily experience building with AI tools such as Claude Code or similar • Able to review generated code against domain and security invariants • Security-conscious when handling PHI • Strong written communication for incident write-ups, runbooks, and decision rationale • US-based with US work authorization • Able to hold production and PHI-scoped access; background check and BAA as required • Healthcare experience, HL7v2/ADT feeds, SMART on FHIR, Redox, Epic/Cerner, HIPAA, or SOC 2 is a plus, not required • Observability experience is a plus • Basic cloud familiarity is a plus; deep AWS expertise is not required • AWS and infrastructure-as-code experience, including CDK, is a plus • Real-time and telephony integrations such as Twilio or Aircall, or production AI/LLM systems, are a plus • Early-stage startup experience is a plus
