đ Description
âą Monitor RPM dashboards and respond to alerts (blood pressure, glucose, weight, etc.) âą Call patients regularly to assess symptoms and reinforce device usage âą Coordinate follow-up with NPs, primary care providers, and caregivers âą Document patient interactions in EMR and internal systems âą Troubleshoot device issues to ensure patient compliance âą Serve as a primary point of contact for a panel of patients âą Support patient enrollment and onboarding as needed âą Collaborate daily with NPs, intake team, and support staff
đŻ Requirements
âą Active Virginia LPN license, preferred, or ability to quickly obtain âą Strong clinical judgment and clear, professional phone communication skills âą Highly organized and comfortable working across multiple platforms âą Experience with RPM, telehealth, or care coordination preferred âą Interest in preventive care and patient engagement âą Ability to work effectively in a fast-paced, structured environment
