📋 Description

• Deliver patient care services through a remote technology platform • Independently provide holistic, evidence-based care for patients aged 18 months and above • Assess, diagnose, treat, and manage health problems • Provide health counseling and disposition planning • Educate and provide treatment for pregnancy prevention, STI prevention, and safer sex practices • Evaluate primary care, acute, chronically ill, and transitional care patients • Provide contraceptive care counseling and medication management • Educate patients on health maintenance and respond to patient care inquiries • Document all patient care in an EHR according to MinuteClinic policies and procedures • Coordinate patient care with internal and external colleagues and the broader patient-centered medical home • Work effectively within a patient care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice • Work three weekdays and every Saturday and Sunday during the 2:30 p.m. to 11:00 p.m. shift, totaling 40 hours per week

🎯 Requirements

• Current, unrestricted license in one of: CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV • Willingness and ability to obtain additional licensure as needed • Minimum of two years of medically-relevant experience or equivalent • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multi-task • Initiative, problem-solving ability, adaptability, and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Capacity to collaborate with professional colleagues as necessary to provide quality care • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Completion of a Master's Degree level Family Nurse Practitioner program • Current National Board Certification • State of Employment license to practice in the Advanced Practice Nurse role • Broadband connectivity • Quiet setting with a neutral background for remote visits • Ability to uphold patient privacy per CVSH guidelines • Ability to interact with customers remotely, operate computer and phone, speak intelligibly, hear patient responses, and view/read computer screens and electronic devices
