📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review medications and medical histories • Educate members about their health and answer their questions • Help close gaps in care and connect members to appropriate care • Work in a private space with adequate internet connection • Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s) • Engage regularly in manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member experience KPIs and metrics • Provide 40 hours of availability weekly • Maintain 320+ capacity units and 220+ completed visits per month • Conduct Video Visits through telehealth services only

🎯 Requirements

• Full-time, board-certified nurse practitioner or physician assistant • Active state licenses in coverage areas • FNP or AGNP certification preferred for nurse practitioners • Maintain board certification • Able to obtain and maintain multi-state licenses in coverage area, as required • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred • Availability for 40 hours weekly • Consistently provide 320+ capacity units per month • Consistently pace 220+ completed visits per month • Maintain training, affiliation, and compliance with Video Visit requirements • Meet minimum requirements for remote visit delivery, including broadband connectivity with video and audio enabled • Quiet setting with a neutral background for conducting visits • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology
