📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review medications and medical histories during Video Visits • Educate members about their health and answer their questions • Help close gaps in care • Work in a private space with adequate internet connection • Provide 40 hours of weekly availability and 320+ capacity units per month • Maintain a strong visit completion rate of 220+ completed visits per month • Maintain training, affiliation, and compliance with Video Visit requirements • Attend regular manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member experience KPIs and metrics • Deliver telehealth Video Visits only • Practice autonomously while maintaining member privacy and required remote-visit standards

🎯 Requirements

• Nurse Practitioner or Physician Assistant; the posting specifically describes hiring full-time, board-certified nurse practitioners • Board certification required • Active state licenses in coverage areas required • Must be trained and credentialed for all health plan clients and lines of business in primary coverage state(s) • Ability to obtain and maintain multi-state licenses in coverage area, as required • FNP or AGNP certification preferred for NPs • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred • Provide 40 hours of availability weekly • Ability to conduct visits from a private, quiet space with a neutral background and adequate broadband connectivity with video and audio enabled • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Ability to interact with members remotely, sit, write, operate computers and phones, speak intelligibly, and hear member responses • Ability to view and read computer screens, iPads, and other electronic devices
