📋 Description

• Provide appropriate, comprehensive, and proactive onsite and telephonic case management services • Promote high-quality, appropriate, cost-effective healthcare for plan participants with chronic or catastrophic illnesses or injuries • Perform all phases of the case management process under the direction and supervision of a Certified Case Management Professional • Explain the role and scope of activities to patients • Communicate that gathered information will be shared with the payer and obtain consent for case management activities • Assess individual medical and psychosocial needs • Identify issues that may interfere with quality, cost-effective care • Create individualized action plans, set measurable goals, and develop contingency plans • Empower patients to make informed decisions and facilitate communication with the healthcare team • Discuss advance directives when appropriate • Obtain acceptance of plans from patients, families, payers, and providers • Refer patients to qualified, high-quality providers • Regularly evaluate plans for effectiveness, satisfaction, cost-effectiveness, compliance, and quality-of-life impact • Revise plans as medical condition, family status, or insurance coverage changes • Maintain timely, organized, objective, factual, clear, and concise case documentation • Act ethically as a patient advocate while following case management concepts, policies, standards, and guidelines • Become involved as early as possible following illness or injury onset or diagnosis and remain involved throughout the course

🎯 Requirements

• Registered Nurse with a minimum of 5 years Clinical Experience • Credentials such as CCM/CRRN, OCN or other pertinent certifications (preferred) • Excellent written, telephone, and computer skills • Positive, proactive, team-oriented approach/attitude • Time management and organizational skills • Flexible, with the ability to work independently • Active, unrestricted multi-state license • Recent clinical experience • Behavioral Health experience (preferred)
