• Provide integrated care management and utilization management services for members with substance use disorders across the continuum of care • Conduct comprehensive clinical assessments and utilization reviews • Develop individualized care plans • Promote member engagement, recovery-focused treatment, and positive outcomes • Review and process authorization requests • Evaluate medical necessity and determine appropriate levels of care • Facilitate discharge planning • Coordinate referrals to appropriate levels of care and community resources • Partner with members, providers, health plans, and community resources • Address barriers to care through education, advocacy, and coordinated interventions • Maintain accurate clinical documentation and use care management systems effectively • Ensure regulatory compliance • Participate in quality improvement initiatives • Pursue ongoing clinical and industry expertise development
• Current, active, and unrestricted state license issued by a state or territory of the United States to practice independently with a legal scope of practice permitting independent assessments; eligible licenses include Clinical Social Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist, or Registered Nurse • 2+ years of direct clinical care experience in substance use disorder treatment, care management, utilization management, or a related setting • Knowledge of substance use disorders, co-occurring mental health conditions, SUD treatment modalities, and recovery principles • Knowledge of ASAM Criteria and application of level-of-care determinations for substance use disorder treatment • Excellent verbal, written, and interpersonal communication skills • Strong computer and keyboarding skills, including Microsoft Office Suite • Ability to document while simultaneously taking information over the phone • Ability to comprehend medical policy and criteria and clearly articulate health information • Ability to pass background checks upon hire and throughout employment, including criminal, employment, education, sanctions, prohibited-parties, sex-offender-registry, and 10-panel drug-screen checks • Self-motivated and able to ensure objectives are met • Ability to work independently to meet caseload requirements and team objectives • High-speed internet service with minimum 20 Mbps download, 5 Mbps upload, and maximum 100 milliseconds latency • Quiet workspace with minimal background noise • Hardwired internet connection • Ability to sit for extended periods and use wired headsets and desktop equipment
