📋 Description

• Assess, coordinate, and manage care needs of patients with chronic eye diseases • Develop and implement personalized care plans with patients, families, healthcare providers, and internal teams • Manage patient caseloads and communicate according to assessed patient needs • Communicate with patients, families, guardians, caretakers, surrogate decision makers, professionals, home health agencies, and community services • Collect health outcomes data and complete registry documentation • Educate patients and caregivers to support self-management, independent living, and activities of daily living • Assess and support treatment regimen adherence and medication management • Identify community and health resources and facilitate access to needed care and services • Review patient status, laboratory results, and other studies • Develop, communicate, and maintain comprehensive or disease-specific care plans • Use motivational interviewing to identify and address barriers to care holistically • Interact with up to 15 patients per day, providing encouragement, empathy, and support • Meet established goals, metrics, and patient caseload management expectations • Communicate with Team Leads, Managers, Directors, and stakeholders • Perform other duties supporting Lumata Health’s goals and objectives

🎯 Requirements

• 2+ years of work experience in eye care preferred • Active Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) designation required • Proficient with Electronic Medical Record (EMR) systems preferred/required • Ability to work in a team environment with very little supervision • Outstanding written and verbal communication skills • Excellent organizational skills • Excellent customer service, interpersonal, and communication skills • Excellent time management skills with ability to meet deadlines • Strong consultative skills and ability to provide guidance to patients • Ability to work across various time zones and work remotely with reliable Wi-Fi for conferencing and calls • Proficiency with Windows and Google Chrome • Ability to use a mouse, keyboard, touchpad, multiple windows and tabs, File Explorer, screenshots, bookmarks, and browser navigation tools • Ability to troubleshoot basic connectivity, computer, browser, and settings issues independently • Typing speed of 40 or more words per minute preferred • Dedicated private workspace compliant with HIPAA and company security policies • Dedicated reliable internet with at least 50 Mbps download and 10 Mbps upload speed; wired connection preferred • Ability to maintain a low- to-noise workspace and meet call quality standards • Camera must be on during virtual meetings with a professional or neutral background • Ability to sit for extended periods and use keyboard, mouse, and headset for extended periods • Ability to process information from multiple sources, shift between tasks, maintain accuracy with sensitive health information, and follow procedures • Applicants must submit an application with Lumata Health to be considered
