📋 Description

• Conduct regular virtual check-ins to track progress and motivate patients • Support lifestyle, nutrition, and behavior change goals • Serve as a primary point of contact for patient questions and support • Coordinate with providers, dietitians, and care team members • Document patient interactions in the EHR • Escalate clinical concerns to licensed providers as needed • Support patients in the virtual obesity program through ongoing coaching, education, and care coordination • Drive patient engagement, behavior change, and overall patient experience

🎯 Requirements

• 2–5 years of experience in health coaching, patient navigation, care coordination, or a similar patient-facing role • Interest or experience in obesity medicine, lifestyle change, or chronic condition support • Ability to efficiently use and adapt to EHRs, telehealth platforms, digital communication tools, and workflow tools • Strong organizational skills and ability to manage multiple patient touchpoints • Ability to build trust, motivate patients, and communicate empathetically in a virtual setting • Bilingual Spanish/English • Reliable internet connection • Private, HIPAA-conscious remote workspace • Health coaching certification and/or bachelor’s degree in a health-related field preferred
