📋 Description

• Conduct comprehensive nutrition assessments via virtual/telehealth visits • Provide Medical Nutrition Therapy for obesity and related chronic conditions • Develop individualized nutrition care plans • Deliver nutrition education and counseling using motivational interviewing and behavior-change frameworks • Support patient engagement through clear communication and escalate clinical concerns as appropriate • Collaborate with physicians, NPs, health coaches, and care coordinators • Participate in care planning and case reviews as needed • Document encounters, assessments, and care plans in the EHR • Use telehealth and patient engagement tools effectively • Contribute to clinical program development, quality initiatives, and interdisciplinary care innovation as the organization grows

🎯 Requirements

• Active RD/RDN credential in good standing • Active LD/LDN or CD/CDN license in state(s) of practice • Bachelor’s or master’s degree in Nutrition/Dietetics or related field • Clinical/ambulatory experience providing nutrition counseling or Medical Nutrition Therapy (required) • Experience with obesity/weight management/chronic conditions and telehealth (preferred) • Strong patient-centered counseling and documentation skills • Comfort working in interdisciplinary teams • Comfort with EHRs and telehealth platforms • Reliable internet and a private, HIPAA-compliant workspace • Ability to maintain licensure standards and HIPAA compliance
