📋 Description

• Provide virtual psychosocial assessments and individual psychotherapy through a telehealth platform • Conduct audio-only sessions as backup when patients cannot access video • Deliver time-framed, goal-focused, skill-based therapy using CBT, solution-focused therapy, motivational interviewing, and behavioral activation • Lead and facilitate group therapy and support groups • Provide support and advocacy to caregivers and family members of patients with dementia and serious mental illness • Document treatment response and monitor clinical symptoms in the EMR • Collaborate in daily team huddles • Identify social determinants of health needs for the care team to resolve • Deliver wrap-around outpatient care to adults with serious mental illness, substance use disorders, and dementia as part of a multidisciplinary care team

🎯 Requirements

• Active, unrestricted Pennsylvania LCSW license • Must be able to bill Medicare • Experience conducting psychotherapy via telehealth • Experience screening, assessing, and treatment-planning for common mental health and/or substance use disorders • Working knowledge of evidence-based brief behavioral interventions, including motivational interviewing, problem-solving treatment, and behavioral activation • Confidential home workspace and reliable internet • Computer provided
