📋 Description

• Drive program engagement by communicating the value of Maven’s care coaching model, optimizing opt-in rates, and supporting long-term behavior change • Co-create SMART goals with members, track adherence, adjust care plans, and course-correct within Maven’s clinical program design • Manage a caseload through ongoing messaging, video appointments, and outreach • Follow clinical program guidelines and protocols while using motivational interviewing and behavior modification techniques • Assess member knowledge and barriers and provide personalized health education • Collaborate with providers to translate clinical recommendations into actionable next steps • Remotely monitor member health data and platform activity to identify risks and take appropriate action • Coordinate care across Maven’s digital network, in-person provider networks, and external partners • Participate in and lead efforts to improve quality of care and clinical outcomes • Provide coverage to ensure members receive appropriate care and timely responses

🎯 Requirements

• 1–2+ years of experience in health coaching or clinical work, such as nursing or behavioral health, focused on patient education, motivational interviewing, and clinical outcomes • Experience working with maternity populations and/or individuals navigating family planning or fertility care • Training and experience coaching individuals toward health outcomes, including medication management, nutrition and exercise goals, or blood-pressure management • Flexibility with changes, efficient work to meet member demand, and a growth mindset • Empathetic written and verbal communication with patients and commitment to a positive member experience • Experience achieving patient-outcome KPIs, including opt-in rates, follow-up adherence, and member satisfaction • Ability to learn and adapt to digital health tools and platforms in a fast-changing technology environment • Willingness to use AI tools in daily workflows • Strong telehealth competency, including phone outreach and video calls • Reliable access to high-speed internet • Health coaching certification (NBC-HWC or ICF) is preferred, not required • Start-up or fast-paced environment experience is preferred, not required • Experience coaching through video, text/chat, and phone is preferred, not required • Familiarity with Google Suite, CRM platforms, EHRs, and Zendesk is preferred, not required • Active work authorization in the US
