📋 Description

• Manage a specified caseload of members independently • Create individualized care plans • Coordinate approved medical and social services • Educate members about Medicare and Medicaid benefits • Serve as a primary contact to address and mitigate member questions and concerns • Partner with physicians, providers, and county financial entities regarding care plans • Provide updates on member progress • Conduct telephonic and in-home member assessments • Schedule and manage work based on member availability and regulatory requirements • Travel within the community to meet members in their homes • Perform other duties as assigned

🎯 Requirements

• Must reside in Minnesota and support membership in the state • RN: RN diploma or degree in Nursing (Associate's or Bachelor's) plus a minimum of 2 years' experience in community-based services, OR Bachelor's degree in Nursing with a Public Health Certification plus a minimum of 1 year experience in community-based services • RN: 3–5+ years of experience beyond degree • SW: Bachelor's degree in Social Work • SW: 3–5+ years of experience beyond degree, including 1–2+ years of community-based services • Active, unrestricted RN License or SW License in Minnesota • MnCHOICES Certified Assessor Training (MNCAT) preferred or ability to obtain within 21 days of employment • Public Health Certification preferred, not required • Experience in home and community-based services/case management preferred • Experience working with people with disabilities and seniors preferred • Comfortable working with diverse, low-income populations • Strong knowledge of Medicare and Medical Assistance programs • Excellent interpersonal skills and ability to simplify medical language for non-medically trained members • Ability to work independently, collaborate as a team player, shift priorities, and reprioritize tasks • Strong clinical and documentation skills • Innovative problem-solving ability • Exceptional organizational skills • Willingness to travel within Medica's Service Area • Reliable transportation • Private area of the home designated for a home office • General comfort with technology and proficiency in Microsoft Office Suite • Must be legally authorized to work in the United States; no work visa sponsorship
