📋 Description

• Assign primary diagnosis and procedural codes for designated ambulatory and hospital outpatient settings • Code diagnoses, procedures, and services for emergency departments, outpatient clinics, same-day surgeries, diagnostic testing, radiology, and laboratory services • Review medical-record documentation to identify pertinent diagnoses and procedures requiring code assignment • Accurately assign ICD-10-CM, modifiers, and HCPCS codes for reimbursement, research, and regulatory compliance • Review records for diagnosis and procedure specificity and appropriate hospital and professional-charge reimbursement • Query physicians when documentation is inadequate, ambiguous, unclear, or coding assignments are not straightforward • Keep current with coding and reimbursement reporting guidelines and escalate concerns to the manager • Maintain at least 95% ICD-10-CM and/or HCPCS coding accuracy while meeting productivity standards • Ensure compliance with AHIMA ethical coding standards and official coding guidelines • Follow Memorial Hermann policies, procedures, safety standards, budgetary specifications, and quality-of-service expectations • Complete mandatory and continuing education and maintain skills competency • Support department-based goals and serve as a preceptor, mentor, and resource to less experienced staff • Provide safe, caring, personalized, and efficient experiences to patients and colleagues • Perform other duties as assigned

🎯 Requirements

• High School Diploma or GED required • RHIT, RHIA, or relevant coding certification(s) from AHIMA or AAPC required • Three years of hospital or ambulatory coding experience required for Surgery/Observation • Two years of hospital or ambulatory coding experience required for Emergency Department • Two years of hospital or ambulatory coding experience required for Interventional Radiology • One year of hospital or ambulatory coding experience preferred for Ancillary/Recurring • Experience coding in a level 1 trauma facility/academic teaching facility preferred • Effective oral and written communication skills • Strong knowledge of ICD-10-CM and/or CPT • Analytical skills for interpreting health-record data and assigning appropriate codes • Proficient knowledge of human anatomy, physiology, medical terminology, and surgical terminology • Critical thinking, good judgment, and decision-making skills • Knowledge of coding compliance policies, official coding guidelines, regulatory requirements, and internal policies and procedures • Proficient in navigating a Windows-based application environment • Must maintain 95% or greater ICD-10-CM and/or HCPCS coding accuracy • Must consistently meet established productivity standards • Must adhere to AHIMA Standards of Ethical Coding and official coding guidelines
